Medical Case Management
HIV Case Management services are a range of client-centered services that link clients to health care, psychosocial support and other services, including treatment adherence. These voluntary services ensure timely and coordinated access to medically appropriate levels of health and support services as well as continuity of care through ongoing assessment of the client and affected family members’ needs and personal support systems.

Currently our coverage area serves the following counties: Adair, Audrian, Bates, Benton, Boone, Carroll, Callaway, Camden, Chariton, Clark, Cole, Cooper, Gasconade, Henry, Howard, Johnson, Knox, Lewis, Linn, Macon, Maries, Marion, Miller, Monroe, Morgan, Moniteau, Montgomery, Osage, Pettis, Phelps, Pike, Pulaski, Putnam, Ralls, Randolph, Saline, Schuyler, Scotland, Shelby, and Sullivan.

HIV Support Groups – Virtual
Phoenix Programs, Inc. has developed online groups to support the needs of people who live with HIV/AIDS and to provide safe environments for connecting and sharing. Weekly meetings are held on Mondays at 7:00 pm and Tuesdays at 8:00 am; attendance is not mandatory, simply select the day and time that works best for you.

Zoom Virtual HIV Support Group
Please download and install the free version of Zoom. You have three ways to participate in our virtual HIV support group using Zoom: Video, Audio and Chat. When using any of the three methods other participants WILL be able to see your profile name and your profile picture. You do have the option to use a pseudonym, or a nickname if that is preferable. If you would not like others to see you (if you are using audio or chat) do not upload a profile picture.

You will receive an email invitation to attend the support group from Zoom
— In the email, there will be a link to “Join Zoom Meeting”, Click the link and your browser will open
— If you do not have Zoom installed, it will ask you to install the Zoom App.
— The Zoom app will open and connect you to the virtual support group
